Monthly Job & Site Development (T30 / Form 04)
Activities. The fields to populate include Activity, Type of Contact, Employer, Business/Meeting Location, Summary/Outcomes/Narrative.
Supporting Doc. The fields to populate are Employment Goal, Individual's Assessment, and the Provider Assessment & Recommendations. The VTS (Person Served Timesheet) is now merged so there will only be one invoice.
Also note that the VTS (Person Served Time) is included in the Supporting Doc and will be merged with the Time Tracker times and activities so only one invoice will be necessary,
Job Placement & Tier III (T31 / Form 05)
Activities. The fields include Activity, Type of Contact, SAM, Comment/Narrative, and the IPE Goal.
Supporting Doc.
There are no fields to populate on the Supporting Doc screen for T31.
Reporting
There is no need to include the Job Start or Final Employment Verification reports. The required information from these reports is already included in the standard T31 template report.
The employment information included on the report comes from the Placement screen. When reporting during the "Job Start" (i.e., NOT after final employment verification), use the three signatures Client, Staff, and Guardian. Also note that since OOD now accepts a verbal signature from the Client, there is a place to enter that information.
For the Final Employment Verification, there is an additional place for the staff signature at the bottom of the Placement screen.
Note: once the Final Verification signature has been added, the client signature will not appear on the report.
General Services (T32 / Form 07)
Activities. The fields include Activity, Type of Contact, Employer, and Summary of Contact or Service Outcome.
Supporting Doc. The fields to populate are Service Objectives, Individual's Assessment, and the Provider Assessment & Recommendations. The VTS (Person Served Timesheet) is now merged so there will only be one invoice.
Work Activities & Assessment (T33 / Form 08)
Activities. The fields include Activity, Type of Contact, Employer, Behavioral Indicator (%), Quality Indicator (%), Quantity Indicator (%), Narrative, and Interventions.
Supporting Doc. The fields include Job Tasks Summary, Support Plan,
Provider Assessment and Recommendations, VTS Agreement (Y/N).
Note: There are no Person Served/VTS timesheets necessary because the VTS units are based on the time entered into Time Tracker.
The following services require "Service" to be selected for the Contact Method to get paid for the service AND VTS, and are billed at a daily rate:

The following service requires "Service" to be selected for the Contact Method to get paid for the service, there is no VTS, and is billed at a daily rate:
Job Readiness (School)
On-The-Job-Supports is billed per unit and does not require "Service" to be selected for the Contact Method. VTS does not apply to On-The-Job Supports.
Transportation (T35 / Form 10)
Activities. The fields are Mileage and Activity.
Supporting Doc.
There are no fields to populate on the Supporting Doc screen for T35.
Pre-ETS (T36 / Form 13)
Activities. The fields are Activity, Type of Contact, and Services Activities.
Supporting Doc. The fields are Narrative and Responses.
Summer Youth Work Experience (T37 / Form 16)
Time Tracker Times. The times entered into Time Tracker are used to calculate the VTS units for billing. The VTS hours are no longer entered into a separate form.
Activities. The fields are Activity and Individual Experiences, Performance, and Interventions.
Supporting Doc. The fields are Week number, Business Name, Work Times, Job Tasks, Individual's Self-Assessment, Provider's Overall Summary & Recommendations.
IMPORTANT! When an individual does not show up, or for those holidays that you want to include in the hours offered, you will need to have two times entered into Time Tracker. For example, suppose the individual doesn't show up one day because of an illness. Let's say the scheduled time was 9am to 2pm. You will want to add to Time Tracker a 9am - 2pm time with "No Show" as the Type of Billing, and also a "No Show" activity. This time will be included in the "Service Hours Offered" so that your billing will include these hours. But this time will NOT be included on the report.
Additionally, you will want to add another time for that day of say 9am-9am. with "Unbillable" as the Type of Billing. The activity for this time will also be "No Show", but you will want to comment on why the individual was absent. This is the time that will be included on the report.
Tier I and JD Plan (T38 / Form 06)
Activities. The fields are Activity and IPE Goal.
Supporting Doc.
There are no fields to populate on the Supporting Doc screen for T38.
Reports. The report for Template 38 "Tier 1 and JD Plan" is actually the Job Development Plan. So there is no report on the net for this template. You can go the Job Development Plan screen for the report that will be attached to the invoice.
