To ensure compliance with the CMS Access Rule and changes within DODD's Salesforce system, updates to the OhioISP will go live on the evening of July 17, 2025. These changes include new assessment fields, standardized tools, and additional signature options.
Additional Questions and Level of Care
A new section titled "Additional Questions" has been added to the Discovery Assessment. This section includes Level of Care (LOC) information and 23 new questions.
Note: The system will not prevent submission of the OhioISP if the Additional Questions section is left incomplete—there are no validation errors tied to this section. However, you may still be responsible for completing these questions to meet compliance requirements. Please refer to DODD guidance to understand when and how this information must be documented.
New Assessment Questions
The new "Additional Questions" section of the Discovery Assessment includes 23 new questions. Each question will require either:
a Yes/No response,
a narrative response,
or both.
Many of the questions also include a "Refuse to Respond" option when applicable.
Downloading LOC Information
To retrieve the latest Level of Care data from DODD:
Go to the Discovery Assessment tab.
Scroll to the Additional Questions section.
Click Download LOC Info.
This will pull the most recent Level of Care information directly from DODD's system.
Both the Level of Care Information and the additional assessment questions will appear at the end of the Discovery Assessment on the final PDF document.
New Standardized Tools
Two new tools are now available in the Brittco Library:
AD8 Dementia Screening Interview
Patient Health Questionnaire-9 (PHQ-9)
These assessments can be completed directly in Brittco and added to the ISP using the Attachments section. Scores are automatically calculated and included on the generated PDF.
Note: These forms are not required to submit an ISP. Please refer to DODD guidance to determine when use of these tools is necessary for compliance.
Changes to the Signature Process
Two new fields have been added to the Signatures section of the ISP:

Consent Behavior Changes
Previous Behavior:
Consents were automatically required for team members with one of the following relationships:
Person Supported, Parent/Guardian, or Guardian.New Behavior:
Consents are now only required when Consent Required is explicitly selected for a team member.
Signature Behavior Changes
Previous Behavior:
The Signature Type dropdown appeared automatically for all team members, prompting users to collect a signature.New Behavior:
The Signature Type dropdown will now only appear if the Signature Required checkbox is selected for that team member.
