In this article we will be going over the types of fields in the form builder.
The types of text boxes are listed below in the 'type' option.
Short Text
Short Text is used for fields that only require one word or a brief response. There will be one line to enter information.
Long Text
Long Text is used when more description is needed. A common example of using this would be a 'Comments' or 'Please Explain' portion of a form. There is no limit to how many characters you can insert into a field unless specifically set.
WYSIWYG
A WYSIWYG(what you see is what you get) is a highly customize-able field that allows you greater flexibility with a category, field, or description. The image below shows that you can format the field with bold, italic, underline, bullet points, headers, text in color, and pictures.
Dropdown
The Dropdown type will have an options field appear when selected after naming the field and pressing Enter on the keyboard. To create options, name the options you want for the drop-down field and press Enter. You can create as many options as you would like for the plan. Only one option can be selected when using a dropdown field.
The dropdown type will look like the image below in the preview.
Date
The Date type creates a date field for the plan. A preview of the date field is below.
Radio Button
The Radio Buttons type creates option fields as well.
Below shows what the radio buttons look like. When choosing radio buttons, it is important to know that only one option can be selected.
Checkbox
The Checkbox type is similar to the radio buttons. The difference between the checkbox and radio buttons is that the checkbox type allows for multiple selections whereas the radio button will only allow for one option to be selected.

Demographic
Demographic- this field type is an easy way to bring over an individual's demographic information. It will automatically pull the information entered from the Client tab so you won't have to add these fields manually.
Label
Label- this field is used for static or boilerplate text on a form. Users of the form cannot fill anything in for this type of field. Common examples are the header of the form, descriptions on how to fill out the form, or an 'I agree this information is correct' section before a signature field. You can copy and paste formatting from another document and keep its formatting in the 'custom HTML' portion of the field. The custom HTML allows for extra formatting like in the WYSIWYG field.
Signature
Signature- this field type will bring over a person's signature to allow for a quick and easy signing of a form. The actual signing will take place after the form as been assigned to an individual and can be done using your finger. You can also send out signature requests with the mail icon or upload a saved signature by clicking on the upload icon.
Table
When the 'Table' Type is selected, more options will appear. The description field stays the same but the 'Width' and 'Columns' options will appear. The table can be the full width of plan or can be half, a third, or a fourth of the page. We also have control over the columns in the table. Name the column in the field, press Enter on the keyboard and create another column. Keep doing that until you have the desired amount of columns. Remember you must hit the 'Enter' on the keyboard for the desired field to be created.
You can have any of the field types in a table. A 'Label' field can be useful in the first column if you want the table to have set rows.
Below is a sample of what a table could look like in the plan.
The Width field allows you to control how much space the category or field will take up on the plan. It can cover the full width, half, 1/3, or 1/4 of the page to create columns or to better utilize space.
Repeater
Repeater- The repeater type is just as the name implies. It is meant for repeatable information, as in adding rows to a certain section. What is unique and convenient with the repeater type is that you can put different field types inside the repeater field and it will keep the formatting of those fields. You can 'repeat' the fields in the repeater as many times as you need to.
The delete option for all the types is the red trashcan.
