Since we just built our custom form, we need to publish the form for it to be used.
Once we have our form saved, we need to publish the form. Click the drop down arrow in the black 'Actions' option to find the the 'Publish' option.
When we click 'Publish' the popup below will appear. We can write a note to give more details about the form, who can use, its purpose, etc. Click the blue 'Publish' option to complete the form building process.
When we preview the form we can see what we created. The blue headers are the categories we created and clicking on them will show us the fields inside the categories. We can see view what we have in case there are any edits that need to be made. The description is at the bottom of the field with an explanation of the field.
