Now that the form is built, we still need to create entries for the form. Click the black 'Create New' option shown below to assign a client, staff, or provider to the form.
The popup below will appear. Choose your client(if this is a client form) and Create Form.
Once the information is filled out, we need to save the form. Click the green 'Save' tab.
Click publish in the Actions tab drop down.
The image below will show how to modify the created form. In the top right corner on the Forms screen is a "Modify Structure" option. Click on that option and it will take us to the plan we want to modify.
Here we can edit or preview the created plan shown below.
We can download the plan as a PDF to see what the final version will look like.
